A machine breakdown is already a high-friction event. The maintenance system should not make the first minutes worse by requiring a long administrative form before anyone can start working.
Separate ownership from reporting
A practical workflow can begin with the machine, priority and current state. A technician takes ownership, goes to the equipment and works the problem. The system can keep the job visible without forcing a full closeout immediately.
Machine running is not the same as report complete
Production can be restored before the technician has time to write a useful report. Treating those as separate states makes the workflow better match what actually happens on the shop floor.
Record what matters after control is restored
Once the immediate failure is under control, the technician can record the action taken, parts used, remaining risks and follow-up work. The goal is not less data. It is collecting the right data at the right moment.
